Sample Report: Outpatient Toenail Removal

Subjective: The patient returns today for removal of his left great toenail. PARQ held previously. Patient consented today.

 

Anesthesia: Digital anesthetic block with 2 cc of 1:1 mixture of 2% Xylocaine and 0.5% Marcaine.

 

Procedure: The patient's foot was sterilely prepped and draped. Nail plate was freed with a curette and bluntly removed with straight Kelly hemostat. Penrose drain was applied, followed by application of phenolic acid. At this point, Penrose drain was released, with the kneecap being refilled. Postoperative dressing was applied.

 

Plan: The patient should follow up for any deviation from the anticipated and explained postoperative course.
